Should One Do Phaco Without Viscoelastic?

April 30, 2009

I read in Ocular Surgery News today of a surgeon who does not use viscoelastic. He uses an AC maintainer during capsulorhexis. Endothelial cell loss was no worse when compared to cases done with viscoelastic. I am inclined to believe this small study, especially with non-dense lenses. I remember that Dr. Blumenthal in Israel routinely used an AC maintainer in all his cases- he also was able to express the nucleus intact through a 6mm wound using the pressure exerted by the AC maintainer. I am sure that those who own ASC’s will undoubtedly start to do away with viscoelastics, as they are not cheap. Since I don’t own an ASC,  I’m sticking with it!
