Revised Guidelines For Plaquenil Screening

Amsler grids are no longer recommended by the Academy- use 10-2 visual fields. Also, OCT ( to detect parafoveal retinal thinning) , mERG and fundus autofluorescence are useful. It is also advisable to consider the ideal weight, not the real weight, when prescribing the 6.5mg/kg/day dose (for women,  the “ideal” weight is 100lb at 5 feet, add 5lb for each additional inch). Remember- cessation of treatment can still lead to progressive vision loss due to the long half-life, so be vigilant.
